• Notice of Privacy Practices (NPP): EGFAAC provides patients with an NPP that explains their privacy rights and how the practice uses and discloses PHI in the new patient intake form. The NPP is provided to patients upon check-in and is available on the practice's website.

• Minimum Necessary Rule: EGFAAC must only use and disclose the minimum amount of PHI necessary to achieve a legitimate purpose.

• Breach Notification: EGFAAC must report breaches of PHI to affected individuals and the HHS. A breach of PHI is an unauthorized use or disclosure of PHI that poses a significant risk of harm to the individual.
